Shine On Awards Guidelines

The Shine On Awards recognize the abilities and service to the community of people with disabilities.
 
To be eligible for nomination nominees must not only be a shining example to others through their individual achievements in coping with their particular condition but must also be actively involved in community service for the benefit of others.
 
Nominees need to have demonstrated a significant level of achievement, through their actions and leadership. For example, some of the following:
  • Personal achievement in overcoming the disability within the limits of the disability, recognizing that the type and extent of the disability may, in itself, place constraints on potential for achievement.
  • Membership of and leadership in community groups.
  • Service to the community and a history of community achievement.
  • Other special awards or forms of recognition.

Completion of the nomination form

  • Nominee's signature giving permission to be nominated must be obtained prior to submitting the nomination form by the proposer.
  • After reading and approving of the completed nomination form, both the nominee and the proposer are required to sign and date the form.
  • Information for the purposes of the Shine On Awards, required for assessment and possible citation, is provided by the nominee, the proposer and those in close liaison with the nominee - e.g. in employment and community life.
  • The nominator must ensure that each section of the nomination form is completed.
  • Use accurate, non-emotive, verifiable statements.
  • It would be appreciated if you would include a small photo of the nominee, if possible.
  • Ensure that names and addresses are correctly spelt, and that all details are provided.
  • Additional information (up to 2 A4 size sheets) may be included if desired.
  • Attach to the nomination form appropriate verification of the nominee's disability and service to the community - e.g. references (up to two), and a medical certificate.

Club Notes

A past nominee may be proposed for re-nomination after four years, to allow for achievement of new or extended initiatives.

A Club may propose more than one nominee. There is no cost to the Club.

Closing date for receipt of nominations is 28th February each year.

Note: The presentation ceremony will take place in May each year in each District by rotation.
